Social Security Number Disclosure
The Social Security number you provide in the application will be used by the University to verify your identity. Disclosure is mandatory. This notification is provided to you as required by the Federal Privacy Act of 1974. The University’s record-keeping systems relating to the application were established prior to January 1, 1975, pursuant to the authority granted to The Regents of the University of California under Article IX, Section 9 of the California Constitution. The University is required by federal law to report your social security number and other pertinent information to the Internal Revenue Service pursuant to the reporting requirements imposed by the Taxpayer Relief Act of 1997.
State of California Information Practices Act
The State of California Information Practices Act of 1977 requires the University to provide the following information to applicants for admission who are asked to supply information about themselves:
The principal purpose for requesting the information on this form is to process your application for admission for which positive identification of academic records and test scores is required. In addition, if you are applying for financial aid, positive identification must be determined as well as eligibility for aid. Maintenance of this information is authorized by University policy. Furnishing information that is not designated as voluntary or optional is mandatory. Failure to provide such information will delay or may even prevent completion of the admission process. Information furnished may be used by various University departments for admission and other student-related purposes, such as housing and financial aid. This information will be transmitted to the state and federal government if required by law. Individuals have the right of access to this record as it pertains to themselves. The officials responsible for maintaining the information contained on the application are the Chairperson of the program to which you are applying and the Dean of the Graduate Division.
The main purpose for the information requested is to determine eligibility for admission and financial support.

Furnishing certain information required on this form is specifically noted as optional. There is no penalty if you do not complete optional items. - Mandatory Information
Furnishing information that is not designated as optional is mandatory. Failure to provide this information will delay, or possibly prevent, completion of the application process. - Access to Information
Information furnished on the application or related forms may be used by various University departments for student-related purposes: housing, financial aid, and academic departments. Also, this information will be transmitted to state and federal government agencies if required by law. Individuals have the right of access to this information as it pertains to themselves. - Maintenance of Information
University policy authorizes maintenance of this information.
